Okay, so looking at the websites of different programs, I notice that most schools have their own observation form for the OTR to complete. But how do you go about this when you're applying to multiple schools as I know most are? I wouldn't want to have to take 10 observation forms to one OTR that I'm volunteering with. I know it sounds silly but how does everyone go about this?

Unfortunately its just how the system works. I had 4 different forms for my OTs to complete, 1 for OTCAS and then 3 separate forms for different schools who do not participate in the OTCAS application. The way I went about it is I made sure I was super organized up front that way when I approached the therapist I had all they needed in one concise package. I also made it as seamless as possible for them by providing them with prepaid fedex envelopes so it was easy for them to mail off the materials. Its a pain but believe me they are very used to it and most are more than happy to help.
 
do they have to mail them out or can you take the sheet from them and mail it ourselves with the rest of the materials?
 
that's a good point Prodigious. Not all forms require the therapist to send themselves. I know my OTCAS and NYU applications required the therapist to do it themselves. NYU actually required the therapists (and professor) to access the form electronically through email. THAT was a pain! LOL
 
So does OTCAS have one centralized observation form that work for all the schools that use OTCAS? Sorry I'm not familiar with any of this.
 
@otbenoit - I'm sorry! Yes, OTCAS has 1 form which will accompany your letter of recommendation for each person recommending you. You will register the recommender in the OTCAS system. Then you will print out the Letter of Recommendation form for each person. You will complete your portion of the form, sign it, and then forward it to the person writing your letter. They will then complete the form & sign it, write your letter and send both in an envelope (I think with a signature over the seal, but I could be wrong) back to OTCAS who will then update your online application. The letters will subsequently be forwarded to the programs you have designated. Always check with the program directly to ensure that this meets their requirements and no additional steps need to be taken. Some of these application procedures can be tricky.
 
Thank you very much! I'm a sophomore and I will be looking for OTR's to volunteer with soon. I know it will take me no time to get my application year. Can't wait, hope I get in somewhere.
 
Bummer, I just realized all the Texas schools do not use OTCAS, so I will have to bring them all of those forms, poor OT's. Also, even though I'm a long way off am I allowed to get the observation form from OTCAS now to give them b/c I am applying to some school that use it. I have a school OTR who is looking to get me to start with her soon.
 
I don't think that is advisable since the application re-ups each year. The evaluation form will need to be from the year your application is being submitted. But its worth checking out...
