I spoke with a member of an admissions committee at an MD school. They were telling me that deferrals are uncommon because they are rarely sought out, but the schools are actually veryy lenient on deferring students a year if they have some reason for wanting that. Its just a matter of filling out a form.
Accept the offer. Then bring up the deferral.
Worse case scenario (unlikely), they can't. And you at least have a solid reason for turning down a seat that can be simply explained on your next application (denial of your request for deferral)
