Hello everyone. I am a second year who will write the exam in June 2011. Meanwhile let this be a good thread where everyone share their study progress and recent trend of the exam.

Give the guy a break, man. He's not even 24 hours removed from taking his test, and you're already asking him for details.

EDIT: 9 posts, 9 questions. If everyone used the forum this selfishly, there would never be any answers, just questions. Ironically (and maybe hypocritically), I'm criticizing selfish posts by making a selfish post. . . This is just something that really annoys me.

I've had similar thoughts on other posts I've seen here. People just come here and ask dumb questions they could have just looked up easily on the internet, and never answer any questions or give anyone any input in different threads. Don't feel too bad for writing that, because I'm sure many people have similar thoughts and feelings on the topic.

hey guys! i could use some advice from those who have taken the test already. here is my progressio so far:
4/23 NBME 7 --> 200
4/30 UWSA 1 --> 216
5/7 NBME 11 ---> 221
5/12 free 150 ---> 88% ( 255 on medfriends)..
5/14 NBME 12 ---> 500/221 SAME EXACT score as NBME 11 a week ago.

5 days til exam. my plan was to review all of FA between tomorrow and wednesday and RR blue margin notes. but i wonder if i should try to do more UWorld? i dont know which is better at this point. ive only gotten through about 40% of uworld, have read all of FA once and most of RR.

hey thanks for the call out (no sarcasm) i guess i didn't realize i was being selfish by asking questions but i really do appreciate all the help that i have received from various members in the forum i just got carried away with the stress of the exam being a few days out.

Hey everyone,

I just need some advice on what to do next?

URx about 75% complete random/timed, avg: 79%
5 weeks out UWSA1 - 760/262
4 wks out NBME5 - 247

Last 20 blocks on UW Random timed first time, avg: 84%

I have about 3 and half weeks left and plan on doing FA 2 more times and few more NBMEs. Is it normal for scores to go down from UWSA to NBMEs?

take the test nowwwwwwwwwwwwwwwww

Where's the source for more immuno/diff material after the 17th. I couldn't find any such thing on the official Step 1 website.

go to the "next" page and it is the the last announcement titled "Revised Step 1 Content Outline" under the date November 02, 2010. Hopefully some of the people who take the test this next week can shed some light onto what kind of changes are actually being made. If you compare the 2010 and 2011 Content outlines, Immunology has alot more bullets under it.

interesting.. it looks like the basic stuff that's in FA though.. nothing I wouldn't have expected, but I guess we'll have to see

i dont think the content of the exam is changing esp the immuno questions, just a few more scattered through out...i believe that its just getting its own category instead of just being grouped with micro, similar to the the NBME scoring chart probably.

hey guys.. so er odd question. im probably just being paranoid. i will not be drinking coffee the morning of so i dont have to pee during the exam.. however, is anyone worried about having to go #2 during the exam? LOL... will hte adrenaline just take care of it and not have the gut be as motile (via inhibiting VIP and whatnot?).... anyone have any experience with this? lol should i just buy some imodium or something? hahaha im so paranoid sometimes....

i'm definitely bringing pepto and nsaids to the exam in case of headache/the runs.

pregame #2 is absolutely necessary. i can't perform (in anything: sports, job, thinking, acting like a normal human) with a full trunk. and i'm unbelievably sensitive to pepto/immodium, so i'm trying to get myself on 'schedule' for the 8:30 start time. I won't go into any other details...i'm sure you get the jist haha. Isn't it weird that we all think of this?

Just took Step 1 last Wednesday. This is a brief summary of my experience:

I'm an IMG from the Philippines, graduated last year. I started studying last October for around 4-6 weeks. At that time I was able to read the following books:
First Aid
Lippincott Biochemistry (very helpful, great discussion on sickle cell anemia and other hemoglobinopathies. wish i read this book during med school)
Microbiology MRS
Goljan RR Pathology
Clinical Neuroanatomy MRS

After that I stopped studying, went on vacation, didn't touch anything again until February. I went through the following at a more leisurely pace:
BRS Physiology
Anatomy Road Map
Biochemistry MRS
Free 300 questions from the Kaplan Qbank
UWorld - all questions once

Weeks before the exam I did my second run-through of First Aid, finished my first pass of UWorld, then redid all incorrect questions until I got them right. I also looked up answers to the incorrect questions on my NBMEs with extended feedback.

Here are all my stats:
UWorld average 76% (random, unused, timed) --> improved to 78% after I redid all my incorrect questions
2 months out - UWSA 2 650/248
7 weeks out - NBME 7 570/238 (29 incorrect) - old scale
3 weeks out - NBME 12 640/254 (16 incorrect)
1.5 weeks out (same day) - NBME 11 640/254 (16 incorrect); NBME 6 660/259 (11 incorrect)
5 days out (same day) - NBME 5 650/257; NBME 3 640/254
3 days out - Free 150 97%
5/11 Real deal

I studied my ass off to hit the 260 mark but it just didn't happen. Anyway on the day of the exam I woke up REALLY early (4am), which in retrospect was probably not a good thing since I was left with just 3 hours of sleep and I was fighting to keep my eyes open by the 2nd block. Good thing I brought coffee.

The exam itself was both easier and harder than I expected. I had zero questions on pelvic anatomy. Also there were few questions on immunology, neuroanatomy, and gross anatomy, and all of them were straightforward. There were a couple of questions on lab techniques and genetics that I had never encountered before and had no idea how to answer. There was a question on a renal mass that gave no clues in the stem so you had to base your diagnosis solely on the histopathology slide. There was a question on the standard error of the mean. I didn't expect them to ask us to compute for that, and I didn't remember the formula - I didn't even know that I had to pay attention to the sample size, so I have no idea if my guess was right. Microbiology was all right, but not completely straightforward. I was asked how TB survives intracellularly and I made a very unsure guess which I was later relieved to learn was correct. But I didn't read anything about that during my review, it was just a random factoid I was able to dredge up from my memory of our basic science lectures years ago.

Question distribution seemed very random. I got 2 questions on DiGeorge syndrome just a few questions apart within a block (testing different aspects, but still) and 3 questions on mesenteric ischemia also within one block. I got around 5-10 verbatim repeats from the NBMEs (sorry, can't remember which ones) so it's probably a good idea to do at least form 11 or 12.

I guess I was overconfident with the timing because I had simulated the actual length of the exam thrice and I always finished with plenty of time to spare. On the real deal I marked way more questions than usual and spent much more time on the choices. There was one block where I didn't have time to review all my marked questions, but the rest were OK.

If I could do it over again, I'd have subscribed to at least 2 question banks since everyone who scored 260 and above on this forum seemed to do that. Also I would've started using the review books much earlier, during med school.

Thanks by the way to everyone who posted their experiences previously. I got a lot of good advice here. I wish I could fast-forward 3 weeks into the future... Good thing I'll be occupied with studying for CS and CK. I'll be taking them in the next two months. I'll post an update when I get my actual score.

Since a few people asked: the mnemonics I posted are in FA's endocrine section. At least they are in the 2010? I added a few signals to each as appropriate when they came up in UWorld or Qbank. I am not responsible for the accuracy of my addtions to FA's list, nor do I think they will prove very high yield. 🙂

Gs = GCFLATCHAMP is glucagon, calcitonin, FSH, LH, ACTH, TSH, CRH, hCG, ADH (V2 aquaporins), MSH, PTH.
Gq = GGGOAAAT is gastrin, GnRH, GHRH, oxytocin, ADH(V1, vasoconstrictor), alpha1 adrenergics, angiotensin 2, and TRH.
PETCAT is a list of steroid hormones, though it's admittedly kind of stupid to have a mnemonic for these.
JAK/STAT is the intracellular cascade transmitting signals for GH, prolactin, and most cytokines. There are 2 UWorld questions on these.
Intrinsic tyrosine kinase activity is possessed by the receptors for insulin, IGF-1, PDGF, and other growth factors.

I also loved "qiss and qiq til siq of sqs," though you need to remember SPDHV as well ("Speed have" for me, stupid but it worked). Write them across the top of the page with spaces like so.
--S----P----D----H----V
qiss---qiq--si----qs---qs

Final note, I got zero questions on these on my exam. For me, it was far more useful (by 1 question) to know the downstream mediators of Gq. Others' exam experiences will differ..
I was also asked about the downstream mediators of the phospholipase C pathway. And maybe I'm just green-minded, but I loved the "qiss and qiq until you're siq of sqs" mnemonic. I didn't have any difficulty remembering that at all 😉

re: imodium. best time to take it would be the morning of? ive actually never taken it before in my life haha...

re: imodium. best time to take it would be the morning of? ive actually never taken it before in my life haha...

haha, I've been thinking about this particular matter, too, but I'm going to keep my schedule as normal as I can. A small amount of caffeine to get me started and then probably restroom breaks as needed.

I mean, if you have to take a small break it shouldn't be so bad, right? Just make sure you do your business before you start the exam, right? Thus far I haven't seen any posts from previous test-takers saying "but my bowels/bladder were the biggest problem!":laugh::laugh:

(Weirdly enough, though, I did have to pee like crazy during the MCAT...)

Not that it matters but if anyone else got confused for a bit, GHRH is promoted from Gq (IP3) in FA 2010 to Gs (cAMP) in FA 2011.
wikipedia says both is correct, I shall go with wiki.

this mnemonic never made sense to me. If you just memorize the mnemonics on the bottom of that page (inhibitory is "mad 2s," Gq coupled is"HAV 1 M&M," and everything else is Gs), then you have memorized everything in that above mnemonic.... with the one you posted you have to not only remember the mnemonic but hten also remember which order to put the receptors in...just seems like a lot of unnecssary work. But i guess if it works...

Wow, this is much shorter! And therefore easier for me. Thanks!

Well .... In my case "that" happens before the big event or at most only in the very beginning. Once the event starts (the exam in this case) adrenalin takes over and "that" stops. So at worst I suppose you would lose 15 mins of test time.

Oh, and for the person that was worried about coffee stimulating #1, try tea instead. It's diuretic effect is much much milder.

Hey guys,
So i took Step 1 yesterday FINALLY
Someone said this earlier but it is true, how well you do is based upon your state of mind during the exam, if you got enough sleep, etc.
I again did not sleep the night before the test. I was fine until the last 10 qs in the first block. I start feeling dizzy and thought i was going to faint. I forced myself to make it through that block and then take a break even though i hadnt planned on taking one that early. So then i took a break ate some candy and drank some water and went back in. I felt dizzy during the whole 2nd block also so i decided to lean back as far as possible in my chair and that made me feel better so i took most of my blocks like that. I probably looked really funny but oh well. Anyways, blocks 3-7 were ok and i felt fine. I ran out of time on every single block didnt get to check any of my marked answers.

I feel you. I did a simulated exam so I had zero anxiety and zero sleep issues, but I started feeling exactly like you from my second block. The screen got fuzzy, I couldn't see properly, I felt dizzy. It was bad. It was so bad that even though this was supposed to be like the real exam I ended up cheating on my break time. I think I took close to 3 hours break and that was the only way I was able to finish. (I didn't have issues with timing within blocks though. I'm somehow usually always fast with Qs)

So my biggest concern for test day is how I will manage to sit through the entire thing and have so little break time.

Guys, can I ask you something entirely different? My present issue is being able to focus on studying. I so not like studying that I use every and any excuse to distract myself. Even getting myself to a library is an issue. Any tricks/suggestions on how to discipline oneself? Promises of treats/prizes or punishment does NOT work in my case.

Thanks.

Yeah, I tried to do a full 7 blocks yesterday and I couldn't make it without taking several hours break in between. My goal is to try to do better next weekend on the next 7 hour block. I still have 3 more practice tests to "get it right".

In terms of distractions, I think for me it is key to get enough sleep and to vary up my studying location as necessary. Sometimes it just gets boring to study in the same place 7 days in a row. Go to starbucks, go to another library, study at home in your living room, study with a friend, whatever it takes.

👍👍👍👍👍👍 This SO much. I'm living at home and studying for the time being, and while it's okay, I eventually get cabin fever and need to move my "study spot" so to speak every couple of days (library, etc).

Also I'd disable things like Facebook or Twitter if they're a distraction. I disabled my FB before my dedicated study time and it's probably helped loads.

Watched a movie and talked to friends the day before the test. You will toss and turn the night before so go sleep early! I made sure I had atleast 9 hours of sleep...sure enough I just laid there for 2-3 hours ending with 6 hours total.

And yes, the adrenaline will keep you up day of the exam unless you're insane enough to pull an all nighter

Done that - but you'd be amazed at all the "new" things I've found an interest in that I never even knew existed before

When I switch off my wlan I work a lot better.
location is not a factor for me, I could stay in box-room for the whole day.
Also, something that worked really well for me was writing down exactly how many minutes I worked in every session and adding it up at the end of the day.
Initially without the intention to do x hours but somehow it motivates me to be better than yesterday, even if its working 15minutes longer...give this a try.

I'll try the minute keeping thing, thx.

I did consider disabling internet but then how do you use UW?

I know a lot of people who are disabling their facebook/internet, etc. I don't feel like its that much of a problem as long as you limit yourself.

Try working in 1.5 hour blocks and taking 5-15 minute breaks AWAY from your study area. I find that really works for me, since I get my breaks in but I get 1.5 hours of continuous studying, which is a better learning experience for me instead of taking "mini" breaks during studying which we all know turns out to be like 5-10 minute breaks for every 2 minutes of studying

@ar2388 I don't know about Imodium but I think that's definitely something I will consider! During my last testing experience (MCAT) APPARENTLY, I was so stressed the night before, I gave myself self-induced explosive diarrhea (we're all professionals here right??) during the test I had to cut my physical sciences section short bc I had to go so bad.. so.. just ya know.. fyi

oy.. thats what im afraid of!!! hahaha like its questions 20 and you realize damn! you gotta go bad..

fair point, apart from the Qbanks I can do everything else offline.

Edit - Actually there is a way to disable wwweb connection via browsers without disabling internet altogether for programs that need it. So you could still use UW but would not be able to open web sites. I forgot about this when I posted before.

I knew how to do it on my old computer which was XP but not quite sure on Windows 7. I'll post when and if I figure it out.

About 8 days out and I want to dieee. I just don't want to study anymore. I feel like I'm just glossing over FA and whatever else at this point. I swear I'd take it tomorrow if I could. Just for kicks I checked the NBME site but there were no available dates before mine to bump it up to.

Anyone else having trouble focusing with barely a week to go? I feel like I'm just going to lose information out of my mind the more I sit around.

i found the last week/few days were by far the toughest. They just suck - no way of gettin around it. With so few days left and so much infromation, the chances of you reading something that you didnt already know AND it showing up on your exam are seem pretty slim...but i know man, ive got no advice other than you arent alone in thinking this and you just gotta try and find it in you to power through.

Can you take the test earlier? A lot of my friends who took the test said they wished they took it a week earlier bc they reached a point where they just started forgetting things. Just do Qbanks to keep you focused and motivated. Hang in there 👍

i know the feeling.. i just want to die.. or bang my head against the wall.. i just tlak to myself in my head telling myself to keep reading. im trying to stick to the schedule i made.. 3 days left tostudy... fu*uuuuucccccccccccckkkkkkkk meeeeeeeeeeee

congrats VanBrown.. can i steal your score and call it my own? LOL

the problem is isolation, like tom hanks in cast away. find your wilson maybe? (clearly talking out my @ss) but hang in there and best of luck to everyone taking it this week.

Test is this Friday. Not sure what is left to cover. I've read over First Aid again, and there are still things that I am not picking up on. I am going to spend the next four days going over Intro Pharm, Intro Path, heart sounds, renal path, and maybe some neuroanatomy.

Would it be helpful to listen to Golijan as well?
I listened to a few of his lectures last year. They're funny (I think I'm one of the few people who appreciate his sense of humor) and very good at integrating info but too slow and disorganized for a final review, IMO. I think your plan is good. Maybe also try to redo the questions you got wrong on UW, just to see if you learned the explanations well.

I agree, if you really want to go through what he says in his lectures, there's transcripts floating around on the internet somewhere. I would just read through those. Ultimately I actually stopped listening altogether b/c I got more out of reading the transcripts.

the plus side of forums is honesty...im 3 weeks out till my exam and well i got a 520/224 on uworld SA1. I wanna get at least a 235 does anyone know what to make of this, or have any suggestions for the next 3 weeks. Be honest please!

Well, step 1 = go through test and annotate/understand all the info. I just took this exam today as well and I'm currently (taking a break) from doing that. What I've been doing is looking at my weakest subjects on the score report, then looking at the questions I missed. Usually it's a) i straight up didn't know the answer and need to look at first aid/review book then do some questions on it or b)those tricky bastards pulled a fast one and I knew the material, just not well enough to answer their question-->do more questions. So kind of pick your lowest 3 and work from your weakest subject, in my opinion this is the best way to improve your score for your efforts. Focusing on weaknesses sucks, but if you want to get better that's just how it works out. best of luck to you, despite the fact that you may go to the U😉

Done and done. Good luck everybody! Will post a thorough run through of my experiences/study plan in a few days after I make some serious alterations to my GGT levels and NADH/NAD ratios.

Okay this sounds stupid but where are these free 150 Qs? Can someone post the link? I found free 138Qs but I'm not sure if that's the one...
Also where is the conversion for those 150Qs to a 3 digit score?

Sorry for the second post but how much breast pathology are you guys seeing on the exam? I know it's different per person but I just want an idea since I am super bad at it....

I only got one question on my exam that I can recall. I believe it was something to the effect of, "A woman with a history of radical mastectomy 10 yrs ago presents to the physicians office with painless lumps in her 'upper arm'. What is the most likely cause: (a)-(g)" (listed options were various pathogens and cancer).

Yeah that's the one. google med friends for the percent conversion, but it's likely to grossly overpredict.

reposting from NBME thread...

I've done school-based NBME (2 weeks back) - got a 440/203
I've done ~25% of UWorld - averaging about 65%

I've just started studying for boards and have roughly 7 weeks. Whats the best NBME to do now? I was going to try NBME 5 to give myself an idea on where i'm standing. Should I attempt doing an NBME right now or wait for few more weeks?

I'm aiming for 240+
