USMLE Official 2018 Step 1 Experiences and Scores Thread

This forum made possible through the generous support of SDN members, donors, and sponsors. Thank you.
That is so weird, this exact picture is from NBME 5 block 2 and the answer key is hand foot in that. Thanks for the correction buddy!
Yeah I just did NBME 5 after you mentioned where it came from and the original question stem is super non-specific:
"Look at this picture and tell us what he has, but we're gonna use the same picture but this time he has something else." lol

The answer is HFM for this one btw.

Yes it would be because its the BEST answer of the ones given, notice how they didn't have herpes simplex as an answer choice, because HSV would still be a BETTER answer, and they know that, that's why its not an option.

^This, and I feel like this is how you have to approach all of their questions.

Also, I hate them with the fire of a thousand dying suns for being this way. Just saying.

No wonder it looked so familiar. NBME are some sneaky SOBs.

16 days and 8 hours left till my torture ends. Ive decided not to do a final review of FA using the 2018 books, instead Im gonna still use the 2017 version. The reason is that the 2017 version is soo ingrained into my mind, that when I try to think of an answer to a question, I try to remember where the fact was in that book. If I use the new version, its gonna throw me off. Also, after studying the sleep section in the Neuro chapter, Im 90% sure I might have Obstructive Sleep Apnea lol. Does anyone have summarized review of the nephritic/nephrotic syndromes? For the life of me I can't seem to get questions right regarding those topics.

Check out the sketchy for them maybe. I haven't used their path much but it might be a nice easy way to stick the differences in your memory

1 month until the real deal. I've already taken NBME 13 (awhile ago) and 15 (today). Of the remaining ones, which should I make absolutely sure to do before the real deal? (If not all of them).

Based on my understanding, a 2/26 test date should be next Wednesday (3/21), and I’ll be the one after that. We took early in the week tests which sucks cause our wait is longer haha. Hang in there and good luck!

*Edited to clarify (someone please correct me if I’m wrong): scores are released on the 3rd Wednesday from your test date. If you take the test on a Saturday-Tuesday, your “first” Wednesday will be the Wednesday of the following week, not the Wednesday of the week you took it. If you take it on a Thursday/ Friday your “first” Wednesday is the following week as you’d expect. This is why some people wait “longer”. Also, supposedly your test permit will disappear on the Monday of the week your scores are to be released

Hope this helps people waiting or planning.

Will scores still be released due to the weather? As most places will be closed in the northeast..

Just joining the fun - cant believe the test is so close. Started light studying back in January, will finish classes May 1, and will be taking the test June 9. Exactly 80 days away :scared:

Took my first practice NBME (13) today (March 21) to get an idea where I stand, and score a Gentleman's 200.

Goal is 240, but I'd be satisfied by anything over a 220.

Its neat to see people from my app cycle still hanging around - best of luck @Lannister and @bananafish94

That's how I feel. I think I would be happy with anything over a 220. I'm taking my NBME 13 score to my grave, so you're in a much better position than me when I was 80 days out! 🙂

For anyone that wants to feel good about being average, I got my score today and I’m feeling pretty dang good about being average as ****.

CBSE (12 weeks out): 175
NBME 15 (7 weeks out): 178
UWSA 1 (6 weeks out): 228 —> this is definitely my outlier, even after normalizing the inflated score
NBME 16 (5 weeks out): 196
NBME 17 (4 weeks out): 203
NBME 18 (3 weeks out): 198
NBME 19 (2 weeks out): 205
UWSA 2 (5 days out): 209
UWorld %: 60

Actual score: 229

Honestly, looking at it all laid out now, I feel super blessed to have gotten that score.

I have to say that I could have studied harder and better, but my motivation was lacking and it is what it is. I spent about 8 weeks studying 4-7 hours a day. You should probably study more than that. Anyway, I’m thankful it’s over. Now onto step 2!

I feel so left out right now...

Is there somewhere that lists the topics that are incorporated into Rx's "multi-systems" category? I was saving that for our multi-systems course but I'm running out of questions from the other topics so I'm wondering if I should just add it in now.

Current stats:

M1
Test time: July-August 2019
Goal score: 275+

Completed fall 2017:
Sketchy Micro (gone through every vid and reviewing everyday with Anki)
Sketchy Pharm (same as above)

Goals for winter 2017:
Complete all of pathoma and have it on Anki review
Sketchy Pathology (full version comes out December I think?)
Convert usmle Rx flashfacts into Anki format and get through most of it
Start actively listening to Goljan lectures (one a day until my test)
Get started on usmle rx questions towards the end

Summer 2018:
Complete usmle rx and keep up with Anki reviews of sketchy + Pathoma and Goljan lectures

Fall 2018:
Complete Kaplan q bank
Make my first pass through First AID and annotate lightly. Plan on using PDF version of first aid and annotate with notability.
Rewatch pathoma along with what systems are in the block at my school
Keep up with Anki and Goljan lectures
Take my first NBME

Winter 2018:
Complete first pass of Uworld (timed random)
Second pass through First AID and annotate heavily with Uworld.
Rewatch pathoma along with systems in current block
Keep up with Anki + Goljan
Take a second NBME

Dedicated:
Complete second pass through U world
Take every other available online NBME
Take all of the old NBME's offline (to basically calibrate from U world to NBME style questions)
Keep up with Anki reviews and Goljan

Overall goals:

Stay positive and humble
Practice 10-15 mins of meditation everyday
Stay healthy and develop intrinsic motivations

I'll keep you guys posted as I complete more of the goals I've set for myself 😛
Ambitious AF. How did you do on the MCAT?

Hey All! Long time reader, first time poster. I took step a couple days ago (2/26) and after reading through about 9000 posts on SDN over the last day and a half I thought I’d add my own in case it’s useful to anyone later.

Background:
8 weeks of dedicated, really busted my ass the last 4weeks of it. I’ll share how I studied after I get my score (or now if anyone is interested) but i guess the relevant stuff for now is the practice tests..

•NBME 18 (4wks out) - 225 (had about 15% of UWorld done)

•NBME 17 (3wks out)- 232

•NBME 13 (2wks out)- 225 (had a mini panic attack at this point, but I think it had a lot to do with only getting like 4hrs of sleep a night for about 2wks prior)

•NBME 15 (week before)- 259 (on the night I finished UWorld)

•UWSA1 (2 days before)- 266

•UWorld% (first pass, timed)- 73%

Current/post test feelings:

Feels bad man. Lol. That test is nuts, it felt like basically the most confusing block of UWorld. To be fair, I’m a horrible judge of difficulty on tests. I’m almost always down to the wire on time with practice questions and honestly those two assessments above where I did pretty well I thought I got about 50% correct lol. But all that being said, I feel like I’m in the low 220s. I made some REALLY stupid mistakes (not that I haven’t on everything always lol) and just felt like I completely dropped the ball on this thing. It’s just so hard to gauge this exam though that if I got a 210 or a 260 (yea right lol) I wouldn’t be shocked.

Right now my only real solace is that it seems like everyone walks out thinking they failed, but I feel like I’m gonna be the first guy to drop 50 points from his last two assessments .

Anyway, sorry for the long post! I’m happy to answer any other questions about the test (within reason) or any thing else if it helps anyone! Otherwise I’ll update you all with my score next month- assuming I haven’t broken down, faked my own death, and fled to Mexico by then. Lol.

Real deal:
251

Not a score to write home about by SDN standards but considering that I felt like I tanked this one, and knowing that this keeps all my doors pretty wide open... I couldn’t be happier!!!

I posted all my practice tests and general post-test thoughts in the post I quoted above, and a few after that I talked about the resources I used if anyone is interested so I won’t reiterate unless anyone is curious. Basically I just wanted to make a post with all the info I kept trying to find on people to satisfy my neuroticism before the test so I apologize if this is a little random.

•so first of all, I worked my ass off studying. A lot of people talk about how studying for step isn’t that bad or they did a clean 40hr weeks and got a 260-whatever, and that’s awesome for them, but I want to reassure anyone who’s questioning themselves that I don’t consider myself any type of freak genius and I think a 250+ is something you can get with a lot of hard work. I put in 16hr days 7 days a week my last 5wks but I don’t regret a second of it now.

•CBSE:
I got a 195 or something a couple weeks before dedicated and read about a thousand old posts saying that it’s rare to improve more than 30pts from there. BS. The CBSE means nothing imo unless you’ve already done tons of dedicated studying (and I mean tons.. I was about 2/3 through Rx).

•NBMEs:
Don’t psych yourself out too much reading other people’s scores online. It never occurred to me until recently that when you see somebody posting that they got a 250 6 weeks out that they MIGHT have already done UWorld before that point. Maybe not, but i don’t think you can start putting a lot of weight in these until you’ve actually gotten through a good amount of your study plan.

•Resources:
Everyone says this but I’ll reiterate- less is more. I didn’t use a lot of what people claim are essentials but I knew the high yield stuff really well I think. I had never heard of Zanki until a couple weeks ago, watched one SketchyPharm video, and only did about 1/3 of sketchy micro (do kinda wish I’d found time for it).
I’m not knocking any of these resources, but what I never kept in mind is that you do need to know as much as you can, BUT there’s only 280 questions. I felt great on the micro and I think everyone I had was one I know from dedicated.

•how you feel post step:
Well, I felt like crap, and was pretty sure I bombed. I know everyone says everyone feels that way but here’s the thing- I think I actually DID choke a bit. Looking at my breakdown it looked a lot like the NBMEs I totally choked on. The key is though I worked really hard and prepared well enough that, while I think I had high 250s and MAYBE a 260 in me, I choked a bit and still pulled off a score that I don’t think is going to hinder me in any way. My point is, if you know your test anxiety is a deal, just prepare hard enough to be able to afford a drop. You got this.

That’s all I’ve got for now.. may add to this for the benefit of future generations later. Thanks for letting me ramble SDN! Hope it helps somebody someday!

Congrats on the awesome score!! My prediction was only 5 points off - not too shabby!

Haha forget about all these SDN overachievers, that's an amazing score! Congrats!

251!! In a training rn but will post my thoughts later today 🙂

Congrats! I hope you enjoyed a bottle of champagne to celebrate after. Great write-up as well. I don’t feel like I had my best day either, so that was good to hear as I wait out my last few days. Good luck on the rest of your career!

As far as I know they should be(but what do I know?). All I can tell you is that there is nothing on the usmle website announcements page that indicates otherwise.

If you were asking about yesterday, obviously you can disregard this message haha

So today I was told that our school recommends taking an NBME practice exam before taking the CBSE. I was kind of surprised by this and was definitely not planning on doing this. Any thoughts on this?

Sorry that happened. Have some friends that did. Either you got lucky on those practice tests and your foundation wasn't that strong, or you got unlucky on the real test, which is very much possible. Mainly you narrowed down your choices to two options and unluckily picked the wrong answer for those questions which are tricky for everyone.

Hey guys,

First off, congratulations to everyone who has made it through! Your stories have been super inspirational and fun to track as I've buckled down for dedicated. Quick question:

Any updates on what's up with NBME 19 ? I've seen a few threads from 2017 about it under-predicting, but nothing in the new year. My school advisor said it was the most accurate, and that NBME 18 is the one that under-predicts by 10ish points. I just took 19, finished with a 90% which correlated to a 238... Not really what I wanted to see 2 weeks before my test (goal is 250).

What are you looking at going into? If you’re from a smaller school, that’s super competitive.

Honestly it depends on person, 19 overpredicted and 18 under predicted for me, BUT the scale on 19 is much harsher and can really under predict. Our data at our school shows that. I think a 238 on it with 2 weeks left and a goal of 250 is pretty reasonable, use UWSA2 to see where you're at next week, that was the most accurate personally.

Uwsa2 underpredicted by 10 for me🙁

I don’t think this is that uncommon - someone correct me if I’m wrong

Just from reading on here over-prediction of scores seems to be more common than under prediction with the practice tests.

How high were you scoring? The higher you go, the more small variations in % correct can change your score.

Smaller school? I don’t think competition changes with class size lol.

But yea 251 is great regardless and is more than sufficient for any field outside of MAYBE plastics or derm (even then it’s fine)

Idk, at our school a 250 is the highest people really get. Also, their mission statement is to keep as many of us “natives” in the state as possible, a 250 from a local kid trumps a 260 from some east coaster.

Paying it forward since these threads kept me sane while awaiting my score report

Background:
Average student here. UFAPS with 8 weeks dedicated. Studied maybe ~10-12hours/day most days....had to bust my ass because I'm not a genius like many people on here . Took some days off randomly though. During dedicated: got through FA ~2 times, UW x1, Pathoma almost x1, Sketchy ~2 times or more (videos x1, pictures x1+). SketchyMicro was definitely crucial for me. Didn't use SketchyPharm or Path. During preclinical I didn't use FA, probably got through >50% of Pathoma, and used Sketchy very sporadically for bacteria.

NBME 17 (5 weeks from exam): 223 (just finished FA x1, UW ~30% done)
UWSA1 (4 weeks from exam): 251 (UW ~60% done)
Free 120 (2 weeks from exam): 82%
NBME 16 (2 weeks from exam): 236 (UW done at this point)
USWA2 (6 days from exam): 258
NBME 18 (3 days from exam): 240

UW% (1st pass, random, timed): 70%

Real deal: 245-250

The real deal was pretty tough...got a string of hard blocks from block 3 until the last block. Very demoralizing lol. The exam definitely hit my weak areas and I made some dumb mistakes, changed at least 3 answers from correct to incorrect, and definitely got at LEAST 15+ wrong from what I remember. Walked out feeling ok, then kept remembering questions that I got wrong and eventually was unsure of where my score would land and got pretty anxious lol. Overall though, I'm very pleased with what I got and glad I'm done with this stupid thing

Oh yea I see what you’re saying, if you’re trying to stay at a home program especially. We give some serious preference in house here as well (we may be at the same school for all I know haha).

At any rate, I’m gunning pretty hard for ortho and definitely feel like I landed where I needed. Have some friends who are ortho residents now that scored similarly (+/- 2-3) and had zero issues interviewing at the best programs in the country (and all the UC schools which is sort of my dream) so I’m definitely feeling good about probably not having any doors closed at this point.
But all that being said I have no idea how things compare in like Derm or Plastics as someone said (at top programs anyway- neither of those have averages much above a 250 any year so I don’t think there’s anything you NEED a 260 to be competitive for despite SDN lore haha)

Wtf would the reasoning on that POSSIBLY be? I’m unclear still on what the CBSE is except for another NBME that only schools can buy. Do you guys have like something attached to your CBSE score to where it carries weight?

I don't see the point. I regret even taking NBMEs at the start of my dedicated period. There are so few questions on the NBME that they aren't too great at telling you your weaknesses (unless there are blatant gaps in your knowledge). I took 3 at the beginning of dedicated and my scores ranged over 30 points.

Now that I have 1 week until my test and my major content review is done, my number wrong has been pretty consistant and I have an actual gauge of where I'm at.

Ok I'm glad to hear this, I thought it was pretty strange. Like if the CBSE counted for a grade or something it might be different, but if the whole point is to tell you where you stand before dedicated then why prepare for it?

I wonder if schools get evaluated on the CBSE somehow. Ours didn’t make a huge deal about prepping for it but they threatened to write professionalism letters in our file if we finished too quickly. Just seems like there’s more to it than it being for our benefit.
