The program wants to be sure that 1. They fill every spot and 2. You’d be a good match. In that order.

If it’s a regional program then they are worried that you will not rank them high if you clearly have no ties. They will likely ask you in the interview why you want to be in that region to see if there’s more to it then you are just applying everywhere. Or if you are overqualified or looking for something more research oriented then you might not come if they are more of a regional program (usually less research oriented). They don’t want to be your “backup”.
Its challenging to understand which programs are more research-oriented vs clinical focused. Its a catch because you don't want to look unappealing to a program because of "saying the wrong thing" on your personal statement. As an IMG you dont really have the option to pick and chose region, research focus, size, etc. I didnt have a lot of exposure to transplant hepatology or advanced endoscopy in my community residency program and I hope to join a fellowship that is able to provide this. To think that some programs with these opportunities will filter me out because of where I did my residency seems unfair because residency selection was only partially in my hands. The disadvantage is real.

I totally get where you’re coming from. I’m just trying to give the perspective of a program to answer a question. There’s a reason why a regional/community program doesn’t just invite the strongest applicants. Also unless you are a trained actor or professional salesman you should not tell a program what you think they want to hear.

I hope one LOR is from their motility department. The head of that department has extremely powerful friends.

Biggest chance is your home school / Residency program.

You might not even get an interview. Start planning for plan B and Plan C.

Go talk to the the head of the motility department ASAP. This is the best advice I can give you.

I have similar stats, but closer to average step scores, except failed step 2 CS(like a fool, didn't study at all). Also mid-tier US MD and mid-tier IM program. Less research with 5 published abstracts/posters, 5 oral presentations(some state-level, 1 national), 1 manuscript submitted and a couple of national awards in GI related stuff. I'm sure I wasted a bunch of money applying. Debated sending an application, but our chief of GI suggested to 'see what happens.' I'll beg some people I know to put in a word for me to skeem some interviews but even if that works, I highly doubt anyone will rank me after a courtesy interview.

I'm sure lower step scores will weed me out of almost all interviews. This sucks.

This one you have to figure on your own regarding the question to ask since your questions are based on program.

3 publications, 3 posters all first author (ACG,DDW,ACP) 4 posters regional ACP
Currently working on 3 reviews, a case and 2 IRB approved retrospectives (mentioned them in the CV)

Applied to 150+

I think I wasted 4 grand applying, any [brutal] honest opinion about my chances? Or more frankly shall I do hepatology year or research?

why not apply all 160 programs? Whats the limiting factor?

To add to this what is your future plan of applying to a program that is pilot (combined) or has advance or transplant or IBD

Coming from Temple or any of the ANMS programs, if the applicant has showed interest in motility they are ranked very high.

Interest in motility is getting involved in research, Doing elective, going to the ANMS meetings or conferences and joining ANMS and putting it on the application.

Motility in my opinion is huge game changer.

One question you can ask is about the equipment they use and if they have motility specialist ? If they say yes. Ask if they allow one month away electives in IBD centers of excellence or one month mini motility fellowships.

One question on IVs asked is: Have you ever had any conflict or interpersonal issues with anyone in the workplace? I have always found this question difficult and tough to answer. How do people answer this?
 
That’s something doing a Chief year could have helped out with

Well I was hired as faculty at my med school. So I'm effectively doing the same type of work. I was still debating on taking the hit and take less than half the pay to do and double the work and ask for a chief title, but that doesn't always get you interviews either. I guess I've some time to figure out my next move while on this cush academic job. But not a fan of gen internal med for a variety of reasons so I'll not sure what I'll do next.

But a whopping zero interviews so far. My GI letter just got uploaded so maybe that'll make a difference.

Everyone has issues with call schedule, clinic and vacation. The most important important factors are to make sure:
1) remember patient care is paramount
2) you deal with issues in a polite and one on one matter
3) out your self in the other persons shoes
4) come to a common ground
5) look at the issue from different angles and perspectives
6) be professional
7) try to see why and how you can help others and your self
8) ask for help
9) never be rude
10) don’t go running to the chief or department head before even trying to fix the issue your self
