I really haven't had any patients to write iForms about, maybe 2 if I get to count them because I followed them basically from their admissions and wrote lots of progress notes. =(
 
Count people you write progress notes on. Count people discussed at case conferences. Count patients for any reason... you need them. I considered it a significant interaction if I thought about the patient in any significant way. Peds encourages you to use patients discussed at professors rounds, etc.

Oh sweet. That sounds like I can use the whacked out mental status exam conversion disorder lady, who I didn't really interview at all, but we discussed several times because her mental status exam was weirder than any I've seen at the VA.

Oh, what if the person has a whole bunch of the primary diagnoses... I can't use them multiple times can I?
 
I hate Iforms. So far none of my patients have had one of the 28 diagnoses on the "medicine" list. So I've been writing them in on the 2ndary line. What a pain in the ass.

I must be on top of my game, cause I've already done that! A URI is close enough to meningitis, right? No? Crap.

I need to figure out what somatoform disorders would be... they're not listed, and I've seen 2. Psych's actually interesting when you're not getting tested on it with stupid tests that are harder for me than path. All the categorization of disorders are confusing still yet though.

Stress reactions might work... hmmm... 😀

FYI, while you're out there doing psych junk, I was helping diagnose radial head fractures. Orthoglass splints are cool!

I think the ortho nurses' desk is going to hate me when they hear the long gaps in the message I left when I got confused telling them to expect a follow-up appointment. That's what I get for not being prepared.

The nurses at the desk like me, or they did when I was up there a lot for my thumb injury. One was like, "Are we going to see you on rotations?" I basically said, "I'm going to try to get ortho as a rotation, and you'll probably see me before, depending on how often I get injured."
 
That would be me, I believe. Fresh mint is the key.

Mint Julep:

Put 4 or 5 mint leaves in the bottom of a double Old-fashioned glass or Julep tumbler, and add 1-2 tsp. of granulated sugar. Muddle (gently mash up with a wooden spoon if you don't have a muddler) the mint and sugar to release the oils from the mint leaves. Fill the glass with crushed ice and add 2 oz. of bourbon whiskey. Stir until the glass is frosted, then top up with more ice. Add a mint sprig for garnish, toss in a straw, and you're set. I like to use a ton of mint in the garnish, and I cut the straw short so you really have to bury your nose in the mint to drink it...you wind up getting a wonderful fresh minty aroma that really makes the drink.

Mojito:

Put 7 or 8 mint leaves in the bottom of a mixing glass and add 2 teaspoons of granulated sugar. Add just a splash of club soda and muddle until the sugar is dissolved and smells like spearmint gum. Add 2 ounces of light rum and 1 oz. of FRESH lime juice (squeezed from real limes, not that ****ty RealLime juice you can buy in the little plastic bottles). Shake with ice, and strain into a collins glass filled with crushed ice. Top up with club soda, and add a couple sprigs of mint for garnish. Some people like to muddle 3 or 4 lime wedges with the mint/sugar for their mojitos, but I think you're really just making a minty Caipirissima then.

Damn, now I'm thirsty. It was a good (hot) day to drink either of these today.

Do you ever use simple syrup to sweeten your drinks? ( 1lb sugar in 1 cup boiling water)

If you EVER have to call an attending for anything, BE PREPARED. Some of them will chew your head off through the phone. It's funny to hear their tone change from annoyed to be talking to the med student to interested when you sound like you know what your talking about. You probably won't be able to answer all of their questions, but at least know the patient's info, why you're calling, and what you want from them.

basically my only problem was I totally forgot to have the kid's face sheet at the front of the clipboard, so I had to fumble for it in the middle of my phone message to get the parents' phone number off of it. Otherwise I was pretty good. next time I'm writing a script to rehearse first.
 
some of those clips cut pretty deep into the American psyche. I think we can all see a little of ourselves in there.

So, it took a week, but I was told by my patient's daughter that she thought I was 18. Better than the 15 I got at the airport last year.
 
I went to my new apartment yesterday to get my keys. My new landlord knew I wasn't in a huge hurry to move in, so not only did he paint my entire apartment, but he decided that it just didn't look good enough, so he put in new tile in my kitchen, painted the inside of the cupboards, replaced/painted all the molding. He was working hard there yesterday, and was going to yet replace all the molding in my bathroom. If he would've had more time, he would've refinished the hardwood floors (which are in pretty decent condition anyway).

I love my new landlord. And my apartment looks awesome. 😀

Now, if only I had something packed...

When I was working at the hospital drawing blood before med school someone thought I was 12. They didn't want me to draw their blood.

hahaha. That's awesome (and waaaaaaaaay expensive). There was a playground somewhere on the Sunshine Coast in Australia that had one of those, but it was open to the public. I have a picture of me on it.
 
I think I might have finished up my 80% of necessary patient interactions for iforms already this week. God love those poor ICU kiddos and their incredibly complex multi-system conditions.

Little surveys we have to complete on our PDA and send to the administration detailing what kind of patients we have seen on our different rotations. The idea is that they came up with a list of the must-see bread and butter conditions in each specialty and we have to try to see 80% of them in the clinics.

Evidently it's some sort of mandatory reporting for med school licensing or something. Pretty goofy, if you ask me.

Yeah, actually I often use simple syrup in both of those drinks, since you don't have to worry about dissolving the sugar, then. Granulated sugar acts as a bit of an abrasive on the mint, though, so if you really want to be true to form you should use granulated.

I make my simple syrup in a 2:1 ratio of sugar to water...it's more shelf-stable that way, and I like my drinks a little bit sweeter anyway. As long as you're doing that, you should make some homemade grenadine, too. (1:1 sugar and 100% pomegranate juice) It's much better than the high fructose corn syrup artifically flavored stuff you get in the store.

I have til the end of the month to move. Slowly but surely. Just have to figure out when I'm gonna move my furniture. Probably get a uhaul for the day.

And iforms are a pain in the ass. I don't know how I'm gonna see 80% when I keep seeing patients that have diagnoses not on the list. Well, I have outpatient in August, so I'm guessing I'll have more luck then.

So my attending had to go play golf today (purely professional -- some psych dept stuff), and it's pouring rain.

It was pouring in Menomenee Falls, and when my attending called the unit, while I was still there, b/c he told me to stay for a while, he said they were dodging bullets b/c of the weather... hence, pouring.

It also just poured in Tosa.
 
I wouldn't know about weather. You see, I do medicine.
 
A word of advice for the newly minted M2s and perhaps even my beloved classmates: the combo of epocrates on your PDA and a good pocketbook dedicated to the rotation you're on + too much spare time stuck on the wards can make your intern think you're a medical genius.

Yeah, it started pouring pretty hard at 1:45. I just sat in my Jeep for 10-15 minutes until it stopped and then went back to trimming shrubbery.

In OR all day. Got home to find tree parts strewn about lawn. No idea it did anything today.

Oh, sorry, that was me. Thought your shrubbery could use some trimming. Just tell the wife it Funked out today.

I'm just glad a tree didn't fall on my house. A few years ago (during block two of M1 year), one of out trees came crashing down during a storm, missing our house by (not exaggerating here) inches. It did knock off a bit of our corner gutter, but we're lucky it didn't take out our kitchen.
 
By the way, I feel it is our duty to get this thread to >1000 posts ASAP.

I got a piece of carrot cake at Cheesecake Factory yesterday, and I honesslty think I'll be eating it for the next week, if not more. It's the size of my head! Holy Moses.
