Other quick pearls:

You get adenocarcinoma in a urachal diverticulum, meckels diverticulum is from the omphalomesenteric duct, and a fibrous remnant (can't remember the name) of the omphalo- causes volvulus.

Know the Meier-Weigert law for ureteral duplication: the ureteral orifice (UO) upper pole ureter drains medially and inferior to the UO of the lower pole. The lower pole is more likely to reflux, and the upper pole is more likely to be obstructed.

Ashleigh, I heard the NICU is good. But I only heard that from 1 person, and this person thought I was an M4. Apparently I seem knowledgeable...

I start outpatient medicine today. What whirlwind of activity. I had this morning off. After getting my schedule at 8am, I went home and back to bed. Got up at 12:15 with just enough time to shower and make it over to Froedtert for 1pm lecture. Had that til 4pm. Came home. Got on the internet on the unsecured network I found. Which brings me to this moment.

This month is gonna be like vacation compared to last month with inpatient medicine at Froedtert (except for studying for the shelf). I have all Friday afternoons off, and I think Thursday afternoons too. Another Wed morning off. When we have the OSCE I have the entire day off. No weekends. I'm not gonna know what to do with myself.

Finally outta my old apartment. Had to pull on all-nighter Monday night to get it done. 40 hours before I went to bed. My new apartment is much smaller so it looks like a storage space rather than a home right now, and no AC. My dog is panting like crazy.

That's all for now. Still no working TV. 👎

And Don, I freaking love you. Here are the things I need to tell you. Dr. Mateucci is the most amazing resident ever. Dr. Durkee with his surgical mask on looks like Dr. Lombard to me. And next time I see you, I'll tell you all the glowing things they told me about you when I mentioned we were friends. Mike loves you and thinks you're great.

Um, that, and pretty much every other peds subspecialty. Most of them deal with abnormalities of development and how to fix them.

Glad that info helped. One other thing about hypospadias (if you haven't already gotten pimped on it): Severe hypospadias with bilateral undescended testes needs a workup for intersex anomalies.

Yes! Just great. My first patient on neuro has none of the "official" diagnoses (nor is he even close to any of the diagnoses, except maybe trying to prevent some strokes -- s/p carotid stent). This is gonna take some major poetic lisence.

My first lady on neuro was this sweet old lady with an expressive aphasia s/p a stroke 11 years ago. She made some really funny paraphasic errors, and was a really good example of circumlocution when she was trying to name objects. Also, when I asked her what state she was in, she anwered "thirteen."

Try to be a little more open minded and not think in such black and white terms. Patients shouldn't have to conform to your idea of the right answer to avoid being labeled as having a decreased mental status.

i was at least hoping to be in bed by midnight.

but no. right around midnight, some car got smashed into right outside my building. and of course, i was awake and witnessed half of it.

so now i'm up later, having given a report to the cops and now i'm just a little bit scared and paranoid.

thank God I'm moving out of this place soon.

Were you around when some parked cars got smashed outside your building earlier this summer? I think it was July 3 or 4, cuz I stopped over there to drop off my rent at like 6:45am. Was thinking this one car was parked real funny. When I got closer I realized that someone had rammed one parked car into another parked car and drove away. Nice. Doesn't surprise me ... I've got a nice looking dent in the rear passenger corner of my car from parking at Normandy.

So outpatient month is starting nicely. Showed up this morning to find out that my doctor had called in sick. So they paired my up with another doctor, who's new, and is seeing a couple patients for the first time. He made several comments that if I had "something better to do" I should just take off. I said that I don't mind. I'm put in a room to read until a patient arrives. After about 5 minutes, he says that he just wants me to see any of the other doctor's patients that might come in this afternoon, and not any of his new ones. So I'm told to go home, and call at noon to see if I should go back this afternoon or not.

I kinda miss Froedtert, as odd as that sounds.

BTW, my long-time fear of bridges has been justified. I've driven over that bridge so many times...

I never realized before now, since that had a picture, that Jimson Weed is Angel's Trumpet. People in my middle/upper school would use the angel's trumpet that grew on campus.

to get high? that wasn't smart at all. The hallucinations with Datura are reportedly so powerful that you can't tell that you're hallucinating.

If you ever feel bored in Chicago you should hop on the Metra and ride all the way up to Kenosha. Then you'll really know what bored is. You'll also note that the farther north you travel the uglier and fatter the natives get.

Got a letter yesterday saying I had been selected as a student interviewer for next years' admissions. Anybody else doing this?

Ah, the Simpsons. They've portrayed so much of my life in ways I couldn't do myself.

I think it's time to buy a new TV.

Gonna have to go brand new ... I realized that with Craigslist I would need real money. Oops. And I don't really trust any TV selling for $75.

So you're going to finance one? I'd probably just put it on your credit card rather than taking out a line of credit through a store. You really shouldn't need to spend more than $250 for a decent TV from a place like Wal-mart as long as you aren't looking for an HDTV.

Yup, a new TV has to go on the ol' credit card regardless of how much I spend. I live completely off financial aid, which isn't enough to begin with. Don't exactly have extra money just lying around. I don't really have a problem using my credit cards as a temporary fill-in-the-gaps resource until residency.

I looked at some tube tv's, and it just seems silly to get one of those. To get equivalent to what I have, it'll be at least $300. There's hardly any selection too. If I time my sales correctly, I could possibly get an LCD of equivalent size for $600. Which, yeah, it's $300 more, but it's a tv that will likely last longer and will be all set for whatever may come in television. I feel like getting a tube is like putting on a band-aid for the time being.

So I'm debating which is better, getting a band-aid for financial reasons, or get a tv that is actually worth the money I'd be paying for it.


And I have yet to see much of a price difference between stores, i.e. Walmart vs. Best Buy.

Eh, I rather doubt you need an LCD TV. If you don't pay for HD cable, you really don't need an HDTV. Even worse, when an HDTV tries to produce a regular (low-def) signal, it looks even worse than it would on a non-HDTV. DVDs aren't even in high definition (only Blu-Ray or HD DVDs are), so you don't need it for that. If you're going to try to buy something that will be "all set for whatever may come," just go all out and get a TV that shows 1080p (not 1080i or 720p), or otherwise you're going to be using yesteryear technology anyways. If you're going to go HDTV though, I think DLP is the way to go. LCDs get dead pixels and plasmas burn out too quickly.

FYI, all the hype about all television broadcasts converting to digital rather than analog in 2009 as per federal law is only relevant if you don't have cable/satellite.
 
Thanks for all the tips Prowler. It's really not as much about the HD, but more about getting a 30 lb TV, the fact that there's not that much to choose from in tube tv's and an LCD will last longer. An LCD just sounds more worth-while to me than sinking more money into another tube tv. Eh, whatever.

If I really could do it, I think DLP's the best, but those prices are just way too out there.


On a med school related note, outpatient medicine is cake.

dare I say an alternate pathway to emergency medicine involving kids has crept into my mind.
 
oh by the way. Normandy Village was super awesome enough to only give me one day warning that they'll be showing off my apartment to prospective renters tomorrow. So I had to quick clean out all the junk I don't really want random strangers seeing, like family photos and stuff like unpaid bills and all that. At least it got me to start packing.

So in revenge, I was thinking of doing the following:

1. Writing notes on the mirrors about how the apartment blows (though it's not really that bad)
2. Leaving out my Rohen's color atlas of anatomy on the coffee table, open to a nice gruesome photo. Page suggestions? I thought something fairly benign is sufficient, like a dissected pectoral region showing off some great arteries and the brachiocephalic trunk. Facial stuff is probably over the edge.

Now you truly understand why, when I came home at 10pm one night after call to find a note saying they were gonna show my apartment the next afternoon, with a mess of an apartment and not one, but 2 dogs in my care, and an H&P to write up, that I started crying out of pure fatigue.

It's okay, you can cry too.

sadly i'm here now, and they haven't arrived yet, so unless I take off somewhere I'll be here when they show it. I'm seriously tempted to throw in little jabs at the place and piss off the lady showing it.
 
i'm gonna stick with the pectoral region/upper limb.

but as a bonus, i left Robbins open in the bedroom to the photo of toxic megacolon. a little shout out to my nemesis Gimlet.
