Generally, out of how many interviews(assuming they go ok to well), does it take to get at least one acceptance?

Also, do adcomms go over recs, transcript, etc again when they make their decision?

I also wonder if some schools already have in mind that they want to accept you or put you on the waitlist and that the interview is just a formality?

Thanks.

The answer to this question is largely based on the individual. If someone's an awesome interviewer, then maybe almost all to all of the schools will give an acceptance. If someone's terrible at it, then maybe none at all will give an acceptance. However, you can have a great interview and still get waitlisted, so it's not all simple as that.
It depends on the school how much they weigh in the different parts of the application after the interview. Some adcoms only look at the interview once you've been invited. Some only use the interview to identify especially strong or weak talkers, the rest aren't helped or hurt by it.
Yes, it's true that schools invite people to interview knowing full well they'll be accepted if they're not total douche bags at the interview.

1/1, so far I have 3 and expect acceptances from all, just waiting to hear from the other two.

Congrats

I've also had 3 interviews so far and I'm hoping to hear some good news soon *crossing fingers*

I've heard of people having double digit #'s of interviews without getting in. Those people probably had problems with their interview skills. Usually people should get in if they have 4 or more, imho. Usually of course.

That makes me wonder, how can you gauge how well your interview went?

i really think that the interview is weighed differently from school to school. some invite you and then base their decision on your personal skills....while others are completely opposite.....i've had one school interview me and both of the interviewers told me that i had impressed them and they wanted to see me there next year (obviously saying that they wanted me to pick them)......got waitlisted! go figure
