So I've been working with a nonprofit organization that provides volunteer ESL speakers for immigrant and visiting students/scholars in my college town. I recently asked if there was any way to help out beyond having my own conversational partner, and now I'm helping to run the program by assessing new participants' English levels and matching them with volunteer partners.
Does this seem worth the extra time investment? Another thing is that I would now be getting paid for the work in the office (still volunteer conversations though). Would that matter at all when it comes down to application time; and would this be non-clinical work, volunteering, leadership, etc.?

If you want to keep the two components together, you might pick an Activity tag like Teaching, and then explain the two roles within the narrative. Teaching is another valued experience that makes an application more appealing, whether paid or volunteer.

Another positive aspect to this activity is that it involves some level of interpersonal discomfort. It can be awkward to try to converse with people with limited language proficiency, yet it's an unavoidable part of being a physician. Facing that head-on now and learning to deal with it and develop your non-verbal and cross-cultural communication skills is important and worthwhile.
