Just curious for my Navy GMO comrades: Do many EM programs take Navy docs as PGY2's who have done their one year internship and then spent a few years in the fleet as general medical officers (GMO's). Or, do most programs make them start again as PGY1's? I know a few programs make them start again, but they are 3 yr programs.

3-year 1-3 and 4-year 1-4 programs will make you start as a PGY-1. 3-year 2-4 programs will start you as a PGY-2.
Not always the case.
One of the guys in my year is going into a PGY2 slot at a 4 year 1-4 program.
But it's at the program's discretion.

It's not really at the programs discretion. The RRC has requirements for graduation. Unless your PGY-1 year included a large number of the required rotations (unlikely unless it was a transitional year and you picked all the rotations), you can't get credit. Remember, the most credit you can get for an entire IM, FM, or any other residency is only 6 months towards a 3 year EM residency.
Some of the 1-4 programs have both a 1-4 and a 2-4 track, which is likely how your friend is doing it.

Well, yes, that's true... it's at the program's discretion, within the bounds set by the RRC.
But as for credit applied for previous courses of study, that will vary depending on whether the program is AOA or ACGME accredited.
In the course of coming out of internship and heading into residency, I lost my ability to apply for PGY2 spots at a 1-4 program because the programs I was looking at were ACGME accredited and my surgical internship didn't meet their requirements for a 1st year resident. My friend, with the exact same training, was able to get a PGY2 spot in a 1-4 program that is AOA accredited.
So it goes.
