Physician Shadowing Forms

I asked my PCP if I can shadow him for a couple of days. I got a call back from his office staff asking me to come in in 2 days to work out a schedule, and to bring in all my paperwork. I replied "ok, thanks!"
Then I hung up and thought, "aw shoot! What paper work?" 😕 I didn't want to call back and look clueless. I thought I may have to fill out their HIPAA stuff.
I figured I'd need some form of verification of observation form from somewhere.
I've looked all over the net now and can't find anything as far as a generic observation verification form, or something along that line.
Pls help or direct me to where I can find such a form. I'm sure schools may have their own, but I haven't gotten to the stage of selecting schools to apply to yet. Should I just make up my own form?
 
I would call back and ask specifically what paper work they wanted. It would look less silly than showing up with no paper work.

You may want to call them back and ask exactly what they're expecting.
When I shadowed, I had to have proof a double negative TB test and a few other immunizations. Other than that, I signed their paperwork.
 
Yeah, I think I'll just have to call back and "confirm" that I'm bringing all that's required.

Did you have to bring any sort of verification or evaluation-type form? I know he may provide me with an LOR. Maybe he could just include in it something that says I shadowed for a certain amount of time.
As far as the health records, my PCP has his own urgent care, but he's affiliated with the hospital I work at as an RN, so maybe they won't ask me about it, but if they do, I have everything current.
