1. Please state who you are and what the purpose of this survey is.
2. Please provide respondents with a return address (physical or e-mail), since none of that information is presently known.
3. You may want to consider another avenue of data gathering such as surveymonkey.com as it will allow you a much easier route to data collection and analysis.
4. You may want to re-evaluate your intended audience. I'm not quite certain if this forum will yield the numbers or the audience you might be seeking.
