Hello Guys, I have several questions and hope you can help me out, I am very desperate and scared. So around 5-6 days ago when I would take a piss I would feel this poking sensation inside my urethra, kind of like a burning sensation, but very minimal and it would just be when I would start peeing. So I started drinking more water and that went away in 2 days. So the day before yesterday, I felt pain in my scrotum, kind of like if someone was sqeezing it. Also, my testicles were hurting a bit, but very minimal. So this went and the day before yesterday and yesterday. Also, I would feel the poking sensation in my penis, more at the tip, but it would come and go; all these symptoms would come and go. So this morning, I didnt have much pain in scrotum/testicles or much discomfort while urinating, but I had some pain in my left lower back, it would come and go, so I decided to go and see a Dr. So I go to the Dr., tell him all of the above and he does a urinalysis, which a few minutes later he comes back and tells me is negative. It looks like everything is normal' he said maybe I had pulled a muscle and that was causing the pain in my lower back. When he told me this, it came to my mind that a day before all the symptoms started, I was lifting some boxes, I remember one that was pretty heavy and I felt something in my left side of my back when I picked this up. Today after I came from the dr. I was feeling very fatigued and was really nervous. Actually, I have been really nervous since all of these started. So this evening I was feeling that pain in my scrotum again and in my penos, a bit in the testicles, too, but not as bad as before. The pain seems to generate when I am sitting down. So here is my question. Can some nerves from my lower back be causing the pain in the scrotum, testicles, penis? The dr. said the urinalysis was negative, so I really dont know what it can be. I read somewhere that if you stress your lower back muscles, or nerves can cause some discomfort in the areas mentioned. I am really worried, I am scared, I am freaggin nervous, I dont know whats going on. Can any of you give your opinion?

i want to prefacehat i say by saying that i am not yet a student...
did your doctor feel yopur scrotum, do a sonogram? if you are a young guy you at risk for testicular cancer. i have heard people say that test.cancer presents without pain, but here's what happened to my husband. had v.bad lower back pain thought it was from lifting heavy boxes at work. a friend who is a PA said no big deal, take some tylonol, whatever. 2 days later pain goes into scrotum, my husband thinks that it's a hernia, goes to PCP. he looks over and calls in urologist. uro says everything ok go home and rest. PCP who is friend of family says i am still not sure, i would like u to go to ER in just in case. in ER, find out it's test cancer. my point is not to scare u. my husband is actually fine coutesy of some radiation. i just want to relay that somer times doctors overlook things. and if my husband would have listened to the uro, his prognosis could have been worse. so maybe a second opinion?
 
Thanks for the response. Actually She didnt feel my scrotum but I do a self-examination every month or so and dont feel anything diffrent from previous times, so I dont know if its anything to do with that. Whenever I am going to have a BM I feel some typ of pressure in my lower back and the pain seems to start, I dont know if my intestines are pressing on a nerve, or me lifting those heavy things did something like I had said.

RELAX LATINDR...MAYBE YOU PASSED A STONE OR SOMETHING; GET A CONSULT WITH A UROLOGIST.
 
Yep, sounds like you passed a stone...and may be still harboring one or two (or more) somewhere up in your ureters/kidney. I'd get re-evaluated, maybe get an xray of your kidneys (KUB). Though it is no big deal, I'd get it checked because if there are more, then you're likely to have more pain. The fact that you felt relief when drinking more water, and have this pain in your left lower back kind of supports the kidney stone diagnosis. If you leave things alone, then you'll just pass them...but the pain may get worse (or it may not). Anyone in the family with a history of stones??

Let me preface this by saying, you need to go back to your doctor. No one, including myself, can give adequate medical advice without actually laying hands on you. That's why it's discouraged to post personal medical questions on this forum.

That said, let me tell you how I'd approach this if you came to my clinic. (I'm assuming your age is "20-something.")

Physical exam-wise, I'd want to do a few things. First, if she didn't feel your scrotum, she couldn't rule out hernia. This whole "lifting boxes" part of the history would focus my physical exam towards ruling that out, in addition to a thorough lumbar exam to get a sense of whether this pain is related to a slipped disk or something.

Also, and here's where we doctors get personal, she should ask you about your sexual history. Have you been sexually active recently? If so, with a woman, man or both? Do you use condoms? If not, what kind of unprotected sex have you been having? Oral, anal, vaginal, etc? Sounds strange, I know, but she probably shouldn've asked just to rule out an STD or epididymidis, which can occur with certain STDs.

And yes, it could have been a stone as well, though that pain tends to be a little more severe than you described, so much so that inpatient hospitalization is often necessary for IV pain medication. (this all depends on the actual size of the stone, however). Stones can cause radiating pain to the testicle, and some would call this a "classic" finding. Anyone else in your family have stones at an early age?

I would go back to the doctor and get re-evaluated. I'm not sure why "testicular cancer" got posted here, as it's not really at the top of the differential diagnosis list. (Testicular cancer occurs most in this age group, that's true, but cancer of any kind is still one of the last things you'd think of...unless you have other, more worrisome, symptoms. Unexplained weight loss? Lymph nodes? A freaky mass?) Common things being common, you need a reevaluation.

What's more, I'm not certain I would have gotten a Urinalysis with your symptoms. Urinary Tract Infections are pretty rare in men who don't stick things up there, especially in a person who has pretty good hygeine. I'm assuming this because you do self-testicular exams (this implies that you're some what concerned about yoru health, and in can be inferred, about your hygeine.)

Good luck to you, and I hope you have resolution of your symptoms soon.

i wrote about testicular cancer not to suggest rthat he had it- i prefaced that i am not a med student yet and know pretty much nothing. i just wanted to show that sometimes it may take going to more than one doctor... and he should get further checked out

re testicular cancers and " more worrisome symptoms"- when my husband got diagnosed he didn't seem to have that much pain even- just some discomfort in his lower back. otherwise, picture of health- benched 200 pounds, ran, etc. in fact, from his surgeon to the rad onc guy, every doctor remarked how in shape and healthy he was. my point here is that more then one doctor looked at him, and all thought that at worst it may be a little hernia so if your symptoms persist, get checked out further.
 
1. People shouldn't ask for medical advice here. You'll get a premed's answer, or worse.

2. GI Guy's right on target. Testicular CA isn't anywhere near the top of the differential here. It typically presents as a hard mass with very little pain at first.

3. Su4n2, as an aspiring physician, you need to learn an important lesson. Don't throw the word "cancer" around lightly. Words like "cancer" and "AIDS" can scare the crap out of a patient and shouldn't be brought up as a possibility in an initial exam unless they're very likely and you're getting ready to test/make a diagnosis.

4. Hey, GI Man, hope all's well. Just passed ATLS and ACLS. Starting Tuesday. God help my patients.

I started on Tuesday. So far, have been getting out at about 4pm or so, which ain't bad. I still feel like a glorified medical student, only now, the nurses are actually paging me.

"Dr. Future GI Guy, can we stop so and so's fluids...?"

"Sure," I say, not really knowing if she should or not.

Minutes later, I page my senior.

Luckily, it was okay.

Ahhh....the life of a newbie.

Having a stellar senior resident is 90% of the battle, and mine is really good. I almost overdosed a guy on beta-blockers today because I thought he met the criteria for Thyroid toxicosis. Ouch.

By the way, since I'm dealing with veterans, I really only need to understand 5 basic diseases and their corresponding medications (with the government, it's all generic). I'm certain you know what I mean.

Still, mistakes are there to be made.

I passed ACLS, but we don't take ATLS...I guess this is obvious.

3 of my classmates here are from your NEW neck of the woods and they raved about your program. Sounds like you found a good one.

I wish I'd taken those damb medical spanish classes. I hear my friends at the public hospital are getting beaten to a pulp with their lack of spanish knowledge. It all comes back to haunt you.

Hey Future GI Guy and all of you thanks. Actually a day after I posted the Original message, the pain started going away. Now I am feeling "Good" again; like before. There is no more pain in my lower back, so maybe I did passed a stone, or it was a pulled muscle like the doc had said. Thanks a lot for everything.

I am glad you didn't go to the Emergency Room.
You have a Doctor for a reason and his/her workup sounded reasonable enough,
minus forgetting to do a testicular exam.

ED complaints for painful urination: UTI/pyelo, STD, occasionally stone, foreign body?
(yipes) 15% of stones have a normal urinanalysis (i.e. no blood).

ED testicular pain: torsion,STD, epididymitis (sp?....the words throws me for a loop sometimes)

I'm sure there are more extensive lists for these (but I am tired and this is what I mostly
see in the ED ( except for foreign body))

I will likely never officially diagnose tesicular cancer from the ED, but I have seen atleast
one case where the patient had an enlarged testicle and it was highly suspicious. A
pathologist in combination with urology will confirm a suspected diagnosis of Testicular CA.
No test I can do in the ED will give me the final answer. "tissue is the issue"

99% of my renal stone patients go home. most just need hydration and good pain
control. I always check a bun/creatinine to ensure there is little to no hydroneprosis.

A spiral CT scan looking for stones will confirm the diagnosis there. Very few people
order an IVP anymore.

do you jerk off a lot?

a friend of mine would jerk off 3 times a day for 2 weeks, and he had the EXACT same symptoms...

only when his jerking off subsided did he his syptoms go away...
