Not all IMG are created equal. Good programs consider the country of origin as well as the medical center. You should shadow your PI in the clinic if you can't do an externship. Age doesn't matter so much.
The research is important for an IMG, but lots of IMGs are out there doing research. You will need to perform well on the boards to demonstrate that you have clinical facility. Many programs will be concerned about your ability to function on the wards without some sort of graded clinical experience. Now, if you are an IMG from the University of Cambridge, that would help your credibility, but I doubt that is the case here. Also, the time you've been out since finishing medical school is another red flag that you will likely be rusty and might not be able to keep up in the clinical environment.
You should take steps to address each of these concerns to maximize your changes both of matching as well as functioning well as a resident.
