Let your fingers and your good friend Dr. Google do the walking for you. There are so many in a variety of subspecialities so a comprehensive list doesn't exist.

Rather than Google, I would suggest looking at the ACGME website (use "search programs and institutions") or FREIDA. As noted by AdmiralChz above, there is no one list of all combined programs, but you can find a list for each combined specialty at both the sources listed above.

Remember that the ACGME does not (mostly) accredit combined programs, so you must click "All Programs" (not just "Accredited") to see certain lists.
