Programs on Probation

Can anyone shed some light on how to figure out why a program is on probation? The below links list which programs ARE on probation, but I can't figure out why:

I examined the websites of these programs just now. I could find nothing about being on probation on any of the websites. That's awful.
 
Programs can be put on probation for any variety of reasons. If you are concerned about a program you are interested in then you should ask during the interview - and ask how they are working to fix the issues that put them on probation. In fact, expect it to be addressed during the introductory talk by the PD. As an applicant, if you are literally considering spending years at a single institution you need to make sure the program is making positive steps going forward. I know there are a lot of IMGs hanging around SDN, but programs with significant issues should raise a red flag even for you.

It's rare, but program shut downs do occur - I don't have details, but a gen surg program in Daytona Beach, FL was shut down in the past few years and there was a thread recently about a family med program in Mass as well. You need to know these things as an applicant
