Besides UPenn, what programs require a rec letter from internal medicine? Thanks!
 
Dont think there are any others. The only ones that may are the catagorical programs which are few in number. However, its a good idea to have one because prelim and TY programs like to have them and more of them will require it. I didn't personally apply to TY programs but it would make sense they may want one.

A related question -- did you use 4 radonc letters for most programs, or did you have 3 radonc and 1 medicine?
 
No. I had a lot of research that was published so I did 2 rad onc, the surg onc whose lab I worked in while in med school and my PhD advisor. It's very individualized. Use the people who will say the best things about you. If the IM person will say you are in the top couple percent of students then great. Unless a letter will have some real meat your best bet is the most well known name in rad onc.
