My friend (a flight surgeon) got selected for promotion right after he finished his obligation and left active duty. I think he is on IRR now. Can he still pick up the new rank?

I was a LT when I got out in 2000 (7.5 years service). I didn't realize I was still on the IRR books until 3 years later, when I got a letter saying I had been selected for O4. I did not want any chance of being called up, so I quickly checked the box 'turn down promotion and leave the IRR'.

Two months later, I got my honorable Discharge paperwork, and I was listed as LCDR.

Go figure. I'm sure it was an admin mistake, but it does happen.

Most certainly can be and often are promoted, because being a member of the IRR is still considered to be a member of the military.
