Proper Way to Turn Down Acceptances + Withdraw App?

Hey guys,

So I've dug around, and previous threads have said that e-mail or snail mail will do the trick for turning down acceptances. Can anyone kindly suggest a "sample letter/speech" that would sound respectful and professional?

Same goes for withdrawing applications, I plan to call the schools for withdrawals.
