Prostatitis has a tender prostate on DRE. Pyelonephritis has CVA tenderness and WBC casts. UTI has increased urgency, frequency, burning/pain without CVA tenderness & without a tender prostate. BPH is a large nontender prostate sometimes with signs of urinary retention.
