PTCAS Background Info concerns

Hey there,

Under the background info section of PTCAS when they ask "does your academic record accurately represent you academic abilities?", what would be a good approach to answering this if the answer is "no"?

I was basically lazy and didn't study enough for some of my classes, some of which unfortunately were my science pre-req's. How should one go about explaining without making it sound like I'm unable to keep up with the vigorous coursework found in a DPT program?
 
If you were "basically lazy", that's nobody's fault, but yours. If you stopped being lazy, then your academic record should reflect the improvement.
 
If you can find an eloquent way to say you were not motivated in school and have since matured etc etc then do so. Otherwise no.
 
This is a fine line. Unless there's a specific reason or an unusual circumstance, I wouldn't fill this out.
