PTCAS GPA Calculations

Can someone explain to me what I'm looking at? A bit confused.

My GPA splits were calculated on PTCAS, but I'm not sure how to interpret them. I see how they split them up based on subject and such, but which one mostly really matters?

I know it's program-related probably, but is the "self-reported PT Pre-req GPA" the key one?

The Total GPA and the Self reported Prerequisite are the ones that matter most (that and upper division) but most schools calculate their own.
