Up to 5, IIRC. Supposedly an operational billet + deployment = the full 5 points, while a nondeploying spot in a CONUS clinic may not get all 5.
So I have no idea if its changed since then, but here is a detailed description of the point system circa 1998:
Your application is scored by each of the 3 specialty leaders in your chosen specialty on a series of categories:
1. Preclinical Med School years (including step 1) 0-2 points (x3 for a possible total of 6 points)
2. Clinical Med School years (including step 2) 0-3 points (x3 for possible 9)
3. Internship 0-5 points, current interns max at 3 points (x3 for possible 15)
4. Residency 0-5 points (if applying for fellowship, not 2nd residency), current residents max at 3 points (x3 for possible 15)
5. Operational/Utilization Tour 0-5 points (x3 for possible 15)
6. Potential for future success 0-5 points (x3 for possible 15)
Bonus points
0-5 for published research
4 point for prior active duty service (Academy, ROTC, HPSP and USU dont count).
