I have 3 US LORs already.. I have one from my home country from my training officer during a 1 year stint in FM residency.. does the latter count? Is it ok if i submit more than 4 lors ( by sending a new one after the program download a previous lor)? Thanks!

This has been asked several times already, so if you do a search you will find several threads on this. You can send up to 4 LORs unless the program specifically says no more than three. If the 4th one is from a physician who knows you and can provide useful information about how you will perform in a residency, then sure, send it.
You absolutely do not want to play games to send a 5th letter after 4 are downloaded, because it either looks to programs like you cannot follow instructions or are trying to pull a fast one -- it will hurt you more than it would help.
