1) When applying for medical license, I have to mail my medical school a copy of my diploma (which they have already certified). But in general (for this, and all the other stuff I have to mail), is it okay to mail this stuff in a rectangular envelope, or do they require the big envelope so nothing gets folded?

2) Do I need to submit undergraduate transcripts?

Mail it in a regular envelope. You'll only need undergrad transcripts if for some reason they request it. I don't recall ever having to provide that in any of the states that I've been licensed in.
 
It's fine to fold it, but I suggest sending everything you can with some sort of tracking or return receipt. My first medical license was really easy. My second medical license they kept on telling me they hadn't received things that I knew had been sent.
 
License requirements vary with each state. But, it's always safe to send your application with tracking. USPS certified mail is probably the cheapest. I would use a large manilla envelope.
