any general advice on preparation for pain fellowship interviews?

what % of applicants would you say are compromised by PM&R residents?

Are all pain fellowships now ACGME, and thus should theoretically accept other specialties beyond anesthesiology? (even if they dont)

I am from a solid PMR program, and have a first author pub in a highly regarded pain journal... not much other real research, but a strong personal endorsement/letter from a well known anesthesia pain physician

I am applying to fellowships, but wondering if I have a shot at the CC, MGH, texas tech, stanford, mayo.

(my LOR writer does not have connections at those programs)

also, can anyone comment on the vanderbilt program?
 
Don't worry about what your program director sees as great programs, they normally just rec programs they know someone at. Apply to a ton and you will get interviews, and on the interview trail you will find out the differences in programs and what the "word on the street" is from other applicants about strong fellowships. Most programs need to fill a spot with residencies outside of Anesthesia, and they will interview you. Historically some programs only filled with Anesthesia, but those are becoming more a thing of the past as those biased directors are phased out. Good applicants come from all specialties, and if you are strong it won't matter. Top programs look at Step scores, research/publications, interest in pain more than your residency. Go to an ACGME program if you want a solid comprehensive pain education, non-ACGME if you are looking for skills/practice management for the private world.
