So here's a random question. I'm wondering if EM doctors would be able to practice clinical outpatient medicine. I know some ER docs do urgent care. FP docs (in rural areas) work the ER.

p.s. I'm not looking for the answer "why in the hell would and EM doc want to do that" ;-)

Essentially as EM physicians we do practice "clinical outpatient medicine." At least I do on shifts where I admit 1 or 2 patients!!! I think what you probably mean is "Can EM physicians practice primary care?" in which case the answer is easily available by using the search function, conveniently located on SDN's menu bar near the top of the page.

An EP could probably get a away with it and some do. But we're really not trained to manage chronic diseases like diabetes and HTN. If you're in DKA we're all about it. Need a fourth line antihypertensive? Not us.

You mean there's more than one line of antihypertensives?

I just give all the high blood pressure people a prescription for a big bag of nitroprusside and a straw and tell them to take a sip when the pounding starts in their head.

Oh, and keep it out of the light.

I thought that when your blood pressure gets high, you just need some of that medicine. You know doc, that one. I think it starts with a D or something. Well, the last guy I had with high blood pressure said I try some of that on him.

What do you mean you don't know what the white pill is? You're the doctor, you should know!

I always love the suprised look on the pt's face when you don't know what they are taking about when they say "you, know... that white pill... the little one, NOT the big one"

It's also always extremely scary for me to here the pt say "I'm not sure of all of my pills or which one is which, but MY HUSBAND or wife knows" and then to find that their significant other has the IQ of a fruit fly, is color blind and doesn't even remember today's date let alone the meds of his/her SO.

It's like me going to the mechanic and after him asking me "what's seems to be the problem?", me saying "I think it's that plastic or metalic thing inside that's making all of the noise"
