Does anyone know which med schools want LORs to be on their prescribed forms?

If a school has an LOR form, can my recommenders just send a letter or do they have to use the form?

Both of these schools have specific forms that the recommendation letter writers have to fill out in addition to sending the regular letter:

University of Minnesota, Twin Cities
University of Minnesota, Duluth
