Recommendation Letter Structure?

Are there any guide/structure/checklist about the letter of recommendation?

I looked through amcas and aacomas but I dont find anything that tells us what the letter should incorporate.

Is it just "in your opinion, should the applicant be considered to be an md candidate, and why?"

with the majority of the letter explaining the "why" ?
