recommendation waiver for additional letters

If I have my boss write a letter directly to a school that I'm waitlisted at, do I need to fill out one of those waiver forms and have him send it with the letter?

I thought maybe that only applied to letters that got sent through my premed committee. Does the waiver matter at this point?
