regarding LORs

I don't plan on applying to pharm schools until next summer. but i wanted to ask one of my prof. to write a LOR right now before she forgets me. however, since i can't quite start my pharmcas app yet, i can't get the pharmcas reference forms with the specific questions my prof needs to answer. is there a way to obtain that w/o starting the app? 'cause i want the prof to write it before next summer when i start my app.
And same for USC. how do i get the questions for LOR through the usc supplemental app. when i haven't/can't start that yet??

Also, i want to give her my 'stats'/'resume'- something that will tell her more about me. what kinds of stuff should i list or tell her?

She is still going to have to submit her answers through PharmCAS. They aren't questions requiring super long responses, so it is probably better to just ask her now, visit her every once in awhile, and then let her know when the PharmCAS materials are up for that cycle.
