Residency App Process

I have a question about the residency app process, and how it is different for different specialties. I have a long way to go before this is pertinent to me, but I'm just curious.
-When applying to specialties like derm or radiology, do you apply for those residencies right out of med school, or do you have to apply to transitional year programs and apply to the specialty programs the next year?

Most (derm and radiology) programs are applied to dirrectly. Some places even grant an automatic intern year, but that is not standard.

There are chances for straight PGY2 slots for after a transitional year...
