I'm a surgical resident at a well-regarded academic program in the US. Due to scheduling, I have the opportunity to take some time off next year to do a surgical elective. I'm curious about the medical system in the UK and would love to see how the NHS works. Does anyone here have any experience with setting up elective rotations as a resident or have any advice on how to make arrangements?

There tend to be a number of visiting doctors scattered around - some places do have formalised programs. Do you have a particular area of interest - if so, get in touch with the Consultants who are highly regarded in those fields and see what happens.

I am doing a one month rotation in surgical pathology in Scotland in October. Basically it boils down to contacting individual consultants and asking them to have you rotate with them. In order to perform clinical duties you have to be registered with the GMC (General Medical Council), otherwise you can observe.
