Rotation evaluation missing for almost a year

What happens if the rotation I did months ago never sends an evaluation, how will this affect my application? This is beyond my capabilities or my school since my school pestered the doctor who works at his private clinic and he just does not seem to respond, plus he most definitely forgot who I was at this point. Is it normal for residency applications to have a missing evaluation or 2 and do residencies penalize me for it?

Sometimes the answer really is "it depends on the school and so knowing how 10 other schools address the issue doesn't help you when you really just need to know how YOUR school addresses the issue."
