Sending rec letters with secondaries?

I see ppl always saying if you get a letter from an alumnus of a certain school that helps give you an edge. But how does sending the letter work....like do you send out this letter along with all the other letter to all the med schools you're applying to, or only specifically to the school the alumnus went to and ask him/her to specifically mention in the letter that he/she went to that school?

and even if it does do you feel that the rec is kinda throwing the emphasis a little away from your own accomplishments....

im sorry if that sounds rude but many of my distant relatives/friends offered to write on my behalf to med school that they matriculated from. And although I am sure they would write great letters, I just feel like it's really fake.

that's just my opinion....
