SF MATCH and NRMP

They aren't integrated at all.
Yes you can apply to both, but if you match early in neurosurgery via SF match then you are bound to that and have to drop out of the IM match. If you don't match in neurosurgery, then you would go thru NRMP for IM like normal. So all this means that you have to essentially submit 2 sets of applications and letters.
 
Ah okay, thanks for the info. So if one were applying for a transitional year program, how would that fit in?

Do you mean transitional for neurosurg? You would have to apply through NRMP for transitional programs and interview for those as well (which means another set of recommendations and essays!). If you match early in neurosurg, then you can proceed with NRMP and not rank the IM programs. If you don't match in neurosurg, then you can rank the IM programs in NRMP and not the transitional ones. Another thing you can try is to schedule IM interviews after the early match date in Jan. so that if you match neurosurg then you can cancel those interviews and save money...or if you don't match you can still go to them. Maybe you can use a similar strategy for the transitionals. Note not all programs will let you interview so late but it's worth a shot. You will also have to be careful about setting up multiple interviews at the same hospital so I'd recommend applying to one program per hospital (NS vs IM) if you can. It's all doable but it just takes some legwork on your part.
 
Many neurosurg programs include PGY-1 as part of their training. So you'd just upload your neurosurg PS and letters into ERAS, and rank the prelim surgery year at the school where you matched as your only program. And I',m sure you could use the letters and PS, or a modified version, to apply for a separate prelim year as well.

It's unclear from the SFmatch site whether or not there are some programs where you don't have to go through NRMP for the prelim year. It seems to say that, but my dean's office seems to think we have to go through NRMP no matter what.
