Shadowing Multiple Providers

I know this has been asked before, but I've done some searching and I can't find answers to my specific questions.
1) Which provider should I put for the pain information? First chronologically?
2) What information should be included for each new provider? Name, specialty, hospital, email?
3) Do I need to add something more, as in "what I learned" for each provider? Or is it pretty well understood what shadowing is and they just need the information?
I ask this because I shadowed quite a few individual providers and I don't think I'll have room to write something for each one.

1) Chronologically or by hospital system is fine
2) Name, Specialty, Hospital, Contact info, Hours shadowed over which months
3) You don't need to write something for every experience, but you should write a paragraph on what you learned from those encounters, using specific and short example(s) as needed. You want to demonstrate that you were actively learning from your shadowing experience and not just physically there. Every section is an opportunity for the adcoms to learn something about you.

Thank you @Moko. Your answer was super useful. I actual forgot to add the question I was most curious about though lol. For hours shadowed, do I put the total combined hours even though they won't all be with the doctor who's information I'm putting in the beginning?

That's fine as long as it'll be clear from the text

Yes, and be sure you include a subtotal of hours for each shadowed doc in the narrative box. It isn't essential to make an impact statement, especially if space is tight.
