Hi,

I just recently asked a doctor if I could shadow her next semester and she agreed. What forms do I need to get signed/what official documents should I bring to her to make the whole thing more legit?

Contact the office or clinic of the respective physician and ask. Alternatively, if you feel it is appropriate, send an E-mail to the physician to set up a time and go in and shadow. This is much more an experience for you than anyone else. You should not think of this as a box to check; this is how you decide if the career is right for you.

No forms - all function.

Walk in that clinic like you are ready to be the best shadow there ever was. Absorb the light that drips like paint off the pristine white coat of the doctor you shadow. After taking in all you can, open your mouth only when your passion pours out of you like gumbo out of NoLa.

You will be fine, young padawan.

If you need to fill out forms or anything, they'll let you know. I only had to sign a privacy form once for all the shadowing i did.

Why are you asking SDN? We could give you a million and one answers, all of which could and would probably be wrong. Ask the direct source.
