Shadowing Question..

So I know there are a ton of threads about shadowing but this one is rather specific.. I emailed a doctor about shadowing, and in the email I basically told him I'm a third year premed student at college X and I'm very interested in his field of medicine. Well, a few weeks later I get an email from someone at his office asking for my CV and a 'letter of intent'. This is what confused me, can anybody tell me what he means by a letter of intent?? Does he mean a letter of intent to shadow him or what?? What exactly should I say? Something like: Hi My name is blah im interested in blah and i would like to shadow you? Thanks for any help
 
From what I can understand in the context of your situation it is very similar to a personal essay, or statement, difference in terms due to multiple perspective. Anyways, describe your "intent" or why exactly you wish to pursue this particular doctor for that particular specialty. Again this is just an inference, you can always just ask them.

Hmmm, yea the thing is I wasnt planning on making it longer than 5 or 6 sentances so I hope he isnt looking for somkething like a personal statement -____-. Thanks 👍
