Pretty much just drop in or call and ask their receptionist. Worked for me. Sometimes they will say they cant, but most are cool with letting you shadow at least a couple of days.

I sent a letter to all the DOs within a 30 mile radius of me (there were only 12 or so). I knew I'd have to travel a bit to shadow them. In the letter I explained who I was and what I was looking for. Two got back to me, and I ended up shadowing one of them over the course of a few months. I explained upfront that I would be asking for a letter from him. He was very excited about it, and he was very open to helping me with anything. I had called and emailed a bit before, and I had no luck. I figured with a tangible letter, they could respond when they wanted to, and it wouldn't get tossed out in the "unimportant message" pile. They're busy people (everyone in the office including the receptionists/MAs, etc) so just be gracious.

I did what @dkt888 did but expanded my search a little more, as in I sent letter to physicians who are several hours away. If you really want to shadow a DO and want a LOR, which may make or break your chances (at least at some schools that require them), do whatever you need to do (even if it means you have to get a hotel for a day or two because the physician is located far away.)

This site works wonders because all DO physicians should be listed here. It also breaks down and categorizes what specialty they perform in. I would find as many as possibly, and do exactly what @dkt888, @iWillOneDay and @Awesome Sauceome suggests. I personally was lucky in that I volunteered in the PACU ward, developed a good relationship with the nurses, and one of the nurses introduced me to a DO that worked in the hospital. After the initial introductions were made, I have been shadowing him for the past 3 months, and love every minute of it
