Hey!

I was previously accepted to Midwestern IL for the Dental program for the class of 2021. I was not able to accept due to a family emergency that required me to stay close to home. Should I email the adcom telling them i applied this year and why i wasnt able to attend previously so that they don't reject me right away??

Did you try to get an admissions deferral back then?

No i did not as they said it was not an option

I'd let them know to be safe, denying an acceptance might hurt your chances but you did prove they liked you before. They need a reason
