Should I submit a transcript for an interrupted program?

Hello!

I began a post-bac in August 2012 hoping to boost my science GPA for this year's re-application. I was in it for a month before I re-herniated a disc in my back (which rendered me bed-ridden for the next three months until I had surgery) and took a medical leave of absence from the program after meeting with the Dean. I addressed this as best as I could in the 500 character space AADSAS gives you to discuss any interruptions in your academia, but my question is this:

Should I put my course work in to my application and try to submit my transcript from the school, which is just a line of Ws? (That is how they process a medical leave of absence. They also keep your tuition.) I went and took the courses I had been enrolled in during the fall this past spring at community college, so there is data in my application to show that I didn't just quit school, but still. I feel like it is a crap-shoot.
I searched the forum, and it seems as though Ws are NOT included in AADSAS's GPA calculations, so I'm looking for some advice on the matter. I'm finishing up a summer class this week, so I've had to wait for that grade to come in to include it, but it should be ready in the next week or so and I want to submit my app as soon as that last piece is tied up.

Also, should I address this in my personal statement, or is that too melodramatic? It really was a key source of anxiety for me, aside from the pain and the fact that I couldn't walk, to know that this incident was further messing up my already-off-kilter timeline for the application process.

My advice would be to see if the admin at the school you attended would just erase the Ws. You were only there for a month and you had a really good reason to leave.
 
I mean, I was only at that school for the program I left, so I could just not submit a transcript from them at all. I would really only be using it to corroborate the info I included about the academic interruption.
 
Arrange for all official transcripts from all U.S., U.S.Overseas, English-speaking Canadian, and international (foreign) post-secondary institutions attended to arrive at the ADEA AADSAS Transcript Processing Department by the school's application deadline to ensure timely processing of your application (processing time takes up to 4-6 weeks).
