Should I use ranking grid?

hey guys, my school's letterhead has two forms: one without ranking grid and one with.
The ranking grid has a series of questions with the following choices that the writer checks off on:
1. Below average
2.Average
3.Top 25% of those I taught
4.One of the best I taught

Should I include this ranking grid on my letter? I really don't want to, but do most of your undergrad's committee letters have ranking grids?

The schools don't don't specify which one you need. That said, I went with the ranking grids just to be sure.
