I still can't believe this is a thing. For me it sounds like the greatest thing ever since you literally get 6 months of vacation working like this. So far I know Emergency Med, Internist, and critical care all commonly work like this. Is there anything else?

EM, I guess it can be possible. I would not want to work that schedule. Too much ER at one time.

Never heard of that myself.

The main one that comes to mine is a Hospitalist (IM).

Some critical care docs do that too like you said.

Also, some academic docs do their teaching part and then be attendings on floors for a week.

I have heard of one Anes group doing something similar to this as well. I believe they did one week on and off.

Working 80+ hours followed by a week off doesn't sound that great.

I'm not sure that it's always one week off.

For example more commonly what happens in critical care is that you're the ICU attending for one week and then next week you have other duties. It's not like you're laying at home watching ESPN and eating bonbons.

Derm can literally work one day a week and make a living. I have met someone like this who only works Fridays. Kinda sad given that there is such a shortage of derms and the field seems to be attracting primarily women who want to do this kind of part time stuff to raise kids.

I don't know if you could do this in other fields like surgery where skill atrophy might be a bigger concern.

Is there a chart that shows the per hour average wages by specialty? I ideally want to work 2-3 days a week or 1 week on/1 week off in the specialty with the highest per hour wage in which this is possible.

People say you can make your own hours in any specialty but is this true? For example, I don't see many cardiology openings that aren't 8-6 M-F with on call every other weekend

You do realize you need to train in the high dollars/hour specialty for very low dollars/hour for years before any other arrangements are possible, right? Picking a specialty you don't actually like is generally not the best idea.

Hospitalists, em, radiology all do this. Keep in mind that it is 7 day weeks, so you have to include 26 weekends. 26 weekends works out to 10.4 five day weeks, so it's really more like 16 weeks of vacay. As others mentioned, the shifts are typically longer than a traditional work day too, and compensation may or may not be different
