I'm a very CP oriented applicant pursuing a combined residency. I'd like to go somewhere with very strong CP training and research opportunities. Palatable AP rotations would be nice (i.e. there's no amount of PA support that I'd consider too much). At this time my career goals are to work in academics and/or industry. I realize at the end of the day I'm the only one who can decide, but I'd like to hear some input.

Here is my current list in no particular order:

UTSW - Research opportunities. Ability to go CP-only if desired. Flexibility. Loved the PD.
Mayo - National reference center. Has all of my desired fellowships. Liked the PDs and the residents. Hard to say, but I think I like the way they do AP. Cons: So cold!!
Wash U - Great CP. Tons of research funding. Probably my favorite residents Cons: Very long AP hours, wasn't a fan of autopsy here. Didn't feel comfortable in St. Louis.
UPenn - Great CP. Has all of my fellowships. Great residents
UNC - Has all the fellowships I'm looking for. Preferred location Cons: Much less research than the other programs. Facilities seemed somewhat dated.
Duke - Great research, national recognition, gorgeous campus. Lots of PA support. Preferred location. Cons: I keep hearing that CP is on the weaker side here.
 
Imo Penn and WashU are the strongest for both CP and AP on your list... mayo suffers a little from having their own way of doing things. Overall, all are strong programs.

Re: St Louis, it gets a bad rep that is undeserved. The area around BJC is actually very nice and walkable (and livable). Plenty of great neighborhoods within the city, or suburban life is very close by. It certainly is not any more dangerous than Philly, particularly around the hospital. Both are great towns with lots of history and things to do.
 
I will vouch for Mayo. Yes it is cold in the winter, but it will be temporarily. And from the programs you have listed they are the only ones with reference lab - i.e. you will get exposed to everything you ever wanted. Plus Mayo has money , so all the conferences, committees, away rotations and etc. are well covered. Plus national reputation makes easier to score job offers. Plus Rochester is most affordable from the cities listed. Think about it....
 
Yup national reputation is important to score jobs as you mentioned. One of the factors in getting as much leverage as you can in this job market.
