Spontaneous vs Tennsion pneumo

Why doesn't a Spontaneous Pneumo present with tracheal deviation? Won't the trachea eventually deviate away from the affected side like with Tension pneumo? Or should I say will the Spontaneous pneumo eventually become a Tension pneumo??

In tension pnuemo since air that enters the pleural space cannot exit, the expanding pressure of the lung chest/wall combo pushes the trachea towards the opposite side of the thorax. However with a Spontaneous pneumo, rupture of an apical bleb allows air to enter the pleural space but that air is not trapped and is able to leave so there is no deviation. However, you may have a collapsed lung with spontaneous pneumothorax, and in this case you will have deviation toward the side of the collapsed lung. hope that helps
