Dear SDN,
The SMP that I am applying to requires the PS be sent in a sealed envelope. Should I include a cover page? Also, I was thinking of putting it into a report cover in lieu of just sending the single printed page in a manilla envelope. Any thoughts?

Send the single page, no report cover (really no report cover)

You send a cover page if you so desire that should a single line or two . "enclosed is required PS, sincerely name,"
