Here's a quick synopsis of what filled my time off in the summer:
After completion of my freshman year I participated in a 6 week summer program at Baylor Med/Rice Univ that involved shadowing a doc/doing research AND taking 3 classes at Rice (we took a human genetics class, anatomy, and a medical technical writing class). I didn't get to actually shadow a doctor, but I did work in a lab for 6 weeks. At the end of the summer program I was asked to help teach a general chem lab at my Univ for the remainder of the summer. This lab was actually for incoming freshman who wanted/needed a head start with general chemistry.
Going back to my summer experience at Baylor/Rice... my mentor really wanted me to continue working on one of his projects (slave labor I guess), so I had made arrangements with him to continue a particular project while working from my University. This was ideal because it was mostly computer programming and digitizing video, so data was easy to come by - zip disks were cool at the time. I worked on the project for two semesters, and this led to... Another summer program after my Soph. year! I was simultaneously accepted both the SMART and KECK Programs, but I ended up participating with the SMART program (concentrated more on biomedical research). This was a 10 wk long experience, so it didn't leave me with much to do when I got back home.
I then spent the next summer up until graduation working in a biochemistry lab. I also TA'd for a few general biology labs concurrently with my research job.
I guess you can say that I did a bunch of research over the summer AWAY from my school, but most importantly for the medical school that I wanted to go to(I don't think this matters much, as long as you do something). However, I didn't get to participate in any clinical activities such as volunteer work at a hospital etc, but I did get to do those kinds of things during the school year.
My advice to you is to ask your counselor about summer research opportunities (biomedical sciences etc) for freshman. I'm sure there are hundreds of opportunities out there waiting for you -- you just have to find them! Also, if you're truly interested in coming to medical school (i.e. you want to be a physician), then first shadow a doctor and do some volunteer work to get the feel of things.
