Supervision Agreement California - Psychological assistant

I am trying to fill out the supervision agreement SPE form to apply as a psychological assistant.
This agreement plan seems intricate and difficult to write up.
Anyone have a sample, or some advice??
Is this plan ever rejected by the board if it doesn't address every aspect of its requirements?

You supervisor should be filling it out and it is not a good sign if they are not yet informed on what needs to be included. Sit down with them and look at the BOP site together. I believe there are sample supervision agreement forms for psych assistantships available there. If not, try contacting the California Psychological Association for learning what the resources are for examples.
