Supplemental Applications for Elective Rotations

I'm applying for an elective rotation, but they have a supplemental application. They ask me to list my extracurricular activities - which I have already listed in detail on my CV (which has been provided to them) . My question is do I just list the title of my extracurriculars or do i go into detail?
